ACT Psych Nurse (RN)

Primary Purpose of this Position:
The primary purpose of this position is to provide clinical services to all clients receiving ACT team services, including but not limited to; medication monitoring and documentation; delivering psychopharmacologic and medical services within the scope of practice of the individual's license; service coordination, including working with the ACT team to monitor each individual's clinical status and response to treatment; crisis assessment and intervention; symptom assessment and management; individual counseling and psychotherapy; substance abuse treatment; work-related services; activities of daily living services; social, interpersonal relationship, and leisure-time activity services; support services or direct assistance to ensure that clients obtain the basic necessities of daily life; and education, support, and consultation to individuals' families and other major supports.

Description of Responsibilities, Requirements, and Duties:
Overall monitoring of all medical/clinical aspects of the services delivered.
Identify appropriate evidence-based or emerging best practices therapies to be employed by staff in delivering services.
Consult with primary healthcare providers/other providers on the clinical aspects of the clients being served and the implications for those clients' physical healthcare needs.
Coordinate with medical staff of other providers, e.g., state and community inpatient facilities.
Assist in identifying, planning, scheduling, and implementing services for the client.
Be available to respond as needed to ACT team members and collateral medical staff of other providers 24/7/365.
Assist in the development of transition plans when clients move in and out of services.
Provide any written documentation as required. This will include treatment and goal plans and activity notes for the medical record (e.g., service notes, etc.)
This is considered Light Work; no lifting over 20 pounds.
Must be free from all illicit substance use.

Experience / Licensure / Certification:
The registered nurse must be licensed to practice in NC by the Board of Nursing and have at least four years full-time experience in MH/DD/SA with the population served. The registered nurse must have QP status according to 10A NCAC 27G .0104 or be an Advanced Practice Nurse (APN) according to NCGS Chapter 90 Article I, Subchapter 32M.
Each ACT team staff member must successfully participate in the DMH-approved ACTT training. An employee must become NVCI certified and must receive training in population served and client rights/confidentiality. Employees will be required to attend orientation in the Carolina Outreach treatment model and agency policies and procedures. A valid driver's license and current automobile insurance is also required. An employee must demonstrate competence to deliver ACTT services prior to service provision.

Demonstrate ability to join with clients and families in a full partnership building on strengths, community resources, and formal and informal supports.
Possess thorough knowledge of policies, procedures, and standards of care for treatment/services.
Have excellent oral and written communication skills, including presentation effectiveness.
Be competent with office software applications.
Be adept at initiating daily activities without direct supervision.
Have functional and reliable personal vehicle to meet position in-home requirements.
Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
